I want Combat Stress to claim Gift Aid on my donation. This is a personal donation, and I understand that if I pay less Income Tax and/or Capital Gains tax than the amount of Gift Aid claimed on all my donations in that tax year, it is my responsibility to pay any difference.

Please note: Combat Stress can only claim Gift Aid if you provide your full first name, surname and home address. Donations made on behalf of someone else or a group of people – For example, you collect money from your friends and family offline and then donate to your fundraising page are not eligible for Gift Aid.
